Ohio Immunization Summary for School Attendance

According to the state of Ohio, students who have not had the minimum required immunizations may be subject to exclusion. Please review the following schedule of required immunizations from the Ohio Department of Health. You may get immunizations at your child’s health care provider or through the Holmes County Health District. 

If there are medical/religious or philosophical reasons your child cannot receive immunizations, please complete an Immunization Waiver Form.

Diptheria, Tetanus, Pertussis

(Whooping Cough)

5 doses OR 4 doses, if at least one was on or after the 4th birthday

7th Grade:

One dose of Tdap, given on or after the 10th birthday

Polio

4 doses OR 3 doses, if at least one was on or after the 4th birthday

MMR

(Measles, Mumps, Rubella)

2 doses

Hepatits B

3 doses

Varicella

(Chickenpox)

2 doses

MCV4

(Meningococcal Meningitis)

7th Grade:
One dose, given on or after the 10th birthday

12th Grade:
A second dose. If the 1st does was on or after the 16th birthday, a second dose is not required.
